If you’ve been off sick for more than 7 days
If you’ve been off sick for more than 7 days then please request a sick note by:
- Submitting a Sick Note request Form via our Paco GP Digital Front Door (the quickest and easiest way!)
- If you are unable to submit an online request then call our team on 0151 902 0025 between 11am and 6:30pm and they will assist you
- Visit the reception the practice between 8am and 6:30pm
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
