iCare & iShare with the Gold Standards Framework

If you’re living with a life‑limiting condition, our teams will help you plan what matters most. Using the Gold Standards Framework and iCare & iShare, your wishes and clinical plans are recorded once and shared securely with the NHS services that support you. That means coordinated, compassionate care – every time.
How our approach works
- Identify early: we review our lists regularly to spot people who may benefit from extra support.
- Plan together (iCare): we agree treatment goals, preferences, and escalation plans; we can record Advance Statements/Decisions, DNACPR, and Lasting Power of Attorney.
- Share safely (iShare): your plan is visible to your GP team, Community Services, Out‑of‑Hours and North West Ambulance Service so everyone is aligned.
- Review often: you’ll be discussed in our multidisciplinary meetings so actions don’t stall.
What to expect
- A named coordinator and regular check‑ins.
- Conversations about what matters to you (place of care, symptom control, carers’ needs).
- Practical support for forms such as DNACPR or LPA, if you choose.
- Rapid response if your condition changes.
